LOCATION
This position is based at Landow House, our Assisted Living Resident, located at 1799 East Jefferson Street in Rockville, Maryland. The position also provides support to Cohen-Rosen House, our Memory Care Residence. Landow has 53 current residents; Cohen-Rosen has 15.
OUR RESIDENT CARE COORDINATOR POSITION
The primary focus of the Resident Care Coordinator:
- Maintain monthly staffing schedule of all nursing staff.
- Arrange staffing coverage for scheduled absences, and provide assistance with finding coverage for unscheduled absences.
- Maintain updated staff contact list; PRN availability info; records of approved vacation/leave, overtime, and agency use.
- Maintain the Daily Census Report, roster of residents, list of resident family/emergency contacts
- Maintain an up-to-date roster of Residents
- Answer telephone; deliver messages to residents and staff, as necessary.
- Process payroll under the direction of the Director of Assisted Living.
- Complete long term care insurance documentation and submit in a timely manner.
- Keep nursing service staff informed of residents’ appointments.
- Prepare daily resident care assignment sheets - ADL assistance, shower and laundry schedules, and other unit activities.
- Coordinate and track service delivery.
- Oversee the maintenance and distribution of radios, including initiating repair efforts and ordering necessary parts
- Maintain required information for monthly billing chart – move-ins and outs; hospitalizations, service level changes.
- Under the DON’s direction, send letters to family/resident notifying them of service level changes.
- Maintain logs for required staff training and coordinate with HR and DON, as necessary
- Attend and participate in new-hire orientation, workshops/seminars, and mandatory in-service trainings.
- Prepare for new admission: Inform nursing service staff of new admissions, their expected time of arrival, room assignment, etc. Prepare chart with all necessary paperwork.
- Complete necessary medical and administrative records/documentation upon a resident’s admission, transfer and/or discharge.
- Record medical and administrative information in accordance with charting and documentation policies and procedures.
- Record appropriate resident identification data on designated medical records, pictures, ID cards, etc. as required.
- Assemble and maintain charts for all residents in appropriate and chronological order.
- Greet newly admitted residents upon admission. Escort to room as necessary.
- Assist in escorting residents to/from activities and social functions as requested.
- Perform dining room coordinator duties as assigned by the nurse manager.
- Assist in answering residents’ press alerts and setting up trays, as needed.
- Assist in arranging for and scheduling resident health care appointments, including labs, diagnostic and therapeutic services, and physician care.
- Arrange for transportation to medical appointments, as needed. Work closely with the volunteers and staff to ensure timely resident transport to on-site services.
SHIFTS & STATUS
This is a FULL TIME, hourly position on the DAY shift.
EDUCATION & TRAINING
- High school diploma or GED.
- Course of study in medical terminology, or an equivalent combination of education and experience from which comparable knowledge and abilities can be acquired, preferred.
- Computer literate in Microsoft, with ability and willingness to learn new software programs.
- Experience in senior service and health care environments preferred.
- On-the-job training provided in medical records procedures.
- Proficient in scheduling and maintaining staff schedule for all clinical staff members.
- Must be able to read, write, speak, and understand the English language.

VACCINATION POLICY
As a dedicated provider to Senior Living Care our facility requires all hired associates to provide proof of vaccination records, including two doses of MMR, two doses of Varicella, and a Negative TB screen before the start of employment. Another source of documentation that can be accepted is a Titer or Lab test that expresses proven immunity to the following diseases. Upon employment our Talent Acquisition team will provide additional details regarding CESLC vaccination policy.
