LOCATION
This position is based at Wasserman Residence, our post-acute care center, located at 6121 Montrose Road in Rockville, Maryland.


SHIFTS & STATUS
This is an EXEMPT, FULL TIME position on the DAY shift with flexible in office hours between 8AM to 6:30PM.


PURPOSE OF MDS COORDINATOR 
The primary purpose of your job position is to conduct and coordinate the development and completion of the resident assessment and development of the care plan in accordance with the requirements of this state and the policies and goals of this facility.

MAIN RESPONSIBILITIES

  • Works collaboratively and in coordination with the Clinical Team Manager and PI Manager assigned to their Unit to drive quality and financial performance.
  • Conducts and coordinates the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment, including the implementation of CAAs and Triggers
  • Demonstrates basic computer skills.
  • Participates in Utilization Management functions as required.
  • Work with the Interdisciplinary Care Plan Team in developing a comprehensive resident assessment and care plan for each resident.
  • Ensure that quarterly and annual resident assessments and care plan reviews are made on a timely basis.

EDUCATION & TRAINING

  • Registered Nurse from an accredited college, university or program and Licensure to practice as registered nurse in the State of Maryland or under the board of nursing’s guidelines for compact states.
  • Must have a minimum of three (3) years of experience in Long Term Care plus two (2) additional years of experience in clinical nursing.
  • Prior knowledge and experience working with MDS’s.
  • MDS Certification preferred
